10-054 SCOPE: WORLDWIDE Bausch + Lomb is voluntarily recalling the new PreserVision® Eye Vitamin AREDS 2 Formula with Omega 3 soft gels in the U.S. ISSUE: This recall is being initiated as a precaution, prompted by a small number of reports in customers predominately age 70 and older who experienced difficulty swallowing and/or a choking sensation. As a precaution, Bausch + Lomb has decided to initiate a voluntarily recall of this product. INCIDENTS/INJURIES: There have been no incidents reported to date. PRODUCT INFORMATION: PreserVision® Eye Vitamin AREDS 2 Formula with Omega 3 soft gels- 60 Count UPC CODE: 24208 62584 , PRODUCT #: AB62584 EXIPIRATION DATE: 8/31/2011 LOT CODES: 0923BK103, 0924BK103, 0924BK103A, 0925BK103A, 0926BK103A, 0927BK103A, 0928BK103A, 0929BK103A, 0930BK103A. CONSUMER INFO: No other Bausch + Lomb products are affected by this action. To clarify, Ocuvite®, Ocuvite® Extra, Ocuvite® with Lutein, Ocuvite® Adult 50+, PreserVision® AREDS soft gels and tablets, PreserVision® Lutein soft gels are all unaffected by this voluntary recall. The original PreserVision® AREDS vitamin supplement formulation is safe and clinically proven effective in the Age-Related Eye Disease Study (AREDS). Consumers who are presently using the PreserVision® Eye Vitamin AREDS 2 Formula with Omega 3 should consult with their health care professional for an alternative nutritional supplement. While many customers may not experience any difficulties taking the product the manufacturer is asking all users to return any unused product.
